Hope HouseCalls Specialized Dementia Service is our newest caregiver support program providing education, behavioral management, care plan development, home safety plans and cognitive assessments for the memory impaired. It is designed to address many of the issues caregivers experience on a daily basis. The program includes an ongoing assessment of one’s caregiving situation in an effort to develop meaningful and effective interventions for caregivers. We set up a convenient time for our Licensed Social Worker to come to your home. In most cases, fees are covered by Medicare or other medical insurance. Co-pays may apply.
We understand that caregiving for someone with Alzheimer’s disease or a related dementia is incredibly stressful. Through Hope HouseCalls Specialized Dementia Service, and with the assistance of an assigned care manager, caregivers will learn how to best define their needs and receive the help necessary to create a working plan. This one-on-one support also helps caregivers express any concerns they might have about their particular caregiving situation.
